The Coronary Heart Disease Secondary Prevention Resource Pack

Coronary Heart Disease (CHD) is one of the leading causes of death and ill health in the UK.  Secondary prevention is very important in preventing the progression of the disease and evidence has shown that effective clinical management of patients with CHD reduces the incidence of myocardial infarction, cardiac failure and death.

The Resource Pack contains information on all aspects of secondary prevention and includes useful contacts and references to other documents that are relevant to the clinical management of patients with CHD.  The Pack is divided into 6 colour-coded sections and a selection of the pages from each section can be viewed by following the links provided:

1. Introduction

2. CHD Secondary Management - Clinical Management

3. Heart Failure - Clinical Management

4. Diabetes - Clinical Management

5. CHD Secondary Prevention - Lifestyle Management

6. Reference and Appendix

Every practice in Leicestershire & Rutland should have already received a copy of the Resource Pack.  Additional packs will incur a charge and are available using either the form at the back of the Resource Pack or by contacting PCAG for a request form.  The Pack will also be available on a CD ROM.
