New GP Contract and Existing PCAG LV templates(CHD and Diabetes)Oct 2003 Changes The latest Read Code release contains numerous codes that have been created to support GMS2, particularly codes related to exception reporting. This meant a significant re-write of PCAG templates to ensure practices can have access to the codes needed to support the Q&O framework. Practices should decide whether they will use the PCAG templates and / or the EMIS templates as they are released. It may be helpful to read the document 'Guidance for the PCAG Quality and Outcomes Points Estimator' (available in the Q&O points estimator download on this site) before deciding. There are issues of accurate patient identification, what counts as exception coding and the methods of points calculation. The PCAG templates will help to keep practices options open.
The new codes have been added to the templates and a new template and protocol containing the relevant codes is provided too to give a variety of ways to enter the data. The new codes consist of an extended range of codes for drug contraindications / intolerances including clopidogrel, codes for specific exceptions such as maximum tolerated therapy for BP, diabetes, lipids and epilepsy and general exception codes for informed dissent and unsuitable patients. Also included are codes that will help practices to record patient recalls, DNAs, refusals and defaults from follow-up (3 calls with failure to respond).
Flu vaccination has been included in the CHD template along with exception reporting. Codes for NYHA grading have also been released and these have now been incorporated into the heart failure template. It is likely that there will need to be further less extensive code releases next year as GMS2 evolves. There are number of local codes in our templates that probably reflect these gaps.
